Join us for an interactive session where expert faculty will discuss the importance of optimizing guidance-recommended treatment in patients with heart failure at transitions of care.

There are no fees for participating in and receiving CME credit for this online activity.

1. Compare the risks and benefits of optimizing guideline-directed medical therapy during a hospitalization for HF;
2. Evaluate recent clinical trial efficacy and safety data for HF treatment initiation in patients currently or recently hospitalized for HF;
3. Incorporate best practices for the management of patients with HF during transitions of care.
