Overview
The impact of the ever-changing COVID-19 pandemic is creating a rapidly evolving clinical environment for healthcare providers (HCPs). As a result, HCPs are challenged to keep up with the most current clinical information, while constantly evolving their approaches to the management of anticoagulated patients with severe bleeding. As a result, innovative approaches are required to educate these HCPs on how they can make a positive impact on patient care and improve clinical outcomes in the new COVID-19 world, especially as it relates to emergency medicine. 

Patients presenting to the hospital with severe bleeding and underlying treatment with factor Xa or factor IIa inhibitors represent an enormous challenge to emergency physicians, hospitalists, critical care physicians, cardiologists, trauma/general surgeons, hematologists, and pre-hospital care physicians. Intracranial bleeding, intra-abdominal or thoracic trauma, gastrointestinal bleeding, and bleeding from any non-compressible source represent true life-threatening emergencies. Treatment of severe bleeding in patients receiving Factor Xa inhibitors currently involves using non-specific therapy such as blood factor replacement (packed red blood cells (pRBCs), fresh frozen plasma, and platelets). 

With the development and approval of recent reversal agents, treatment of severe bleeding can now be specifically directed at the source of the coagulopathy combined with appropriate factor blood factor replacement. Over the last 18 months, nationally recognized guidelines have provided clear direction on how best to manage these types of major bleeding events. Our assessment of learners within the EMCREG-International/TotalCME network suggests that knowledge and implementation gaps remain. As data emerge regarding new approaches to therapy, clinicians need to be aware of these new and effective approaches and to assess the benefits and risks associated with each of these approaches.

This program, recorded live as a symposium presented during the 2021 EUSEM Congress in Lisbon, discusses therapeutic approaches and reversal strategies of patients on anticoagulation with life-threatening bleeds and how to develop a hospital critical pathway for severe bleeding in the anticoagulated patient.

Objectives
As a result of participating in this activity, participants should be able to:

  1. Describe the indications for anticoagulation and agents including coumadin, factor IIa, and factor Xa inhibitors
  2. Compare and contrast the treatment of intracranial, intraspinal, and gastrointestinal bleeding in the anticoagulated patient
  3. Differentiate and improve care for traumatized anticoagulated patients presenting to the ED requiring surgical treatment and care in the surgical ICU
